Small Estates:

Estate Planning Questionnaire:

To use our time together more effeciently, this form requires advance preparation on your part. Health Care Decisions:

You have the right to control your health care decisions. But if at some time in the future you are unable to express those wishes, there are forms available called Advance Directives. We are happy to provide you with a pamphlet explaining this process as well as the forms that can then be brought to our office, signed and witnessed -WITHOUT CHARGE- Just call for an appointment (503) 357-3191.
